Signed copies of these need to be turned in; two waivers per each child participating. You can download & print your copies HERE, or you can come by and get copies from PCYFC. 

• The original or certified copy of birth certificate is required for each participant. 

• A certified copy is NOT the same as a regular photocopy. Certified copies can be obtained from the register of deeds office in the county your child was born in and can be done online; they will mail it to you for around $10.

• Please visit our FAQ page to learn why this is required to participate. 

• YOU WILL GET THE BIRTH CERTIFICATE BACK AT THE END OF SEASON. 

• Any photo of your child will do; it can also be printed out on regular paper and can be wallet size. 

• a photograph of each participant is part of what we turn in to WNC league to verify participants. See our FAQ page to learn more about the WNC league requirements. 

• A sports physical is NOT THE SAME as a well check. We need a current (given after Nov. 2023) sports physical where a doctor has specifically noted that the child is cleared to play sports. 

• Sometimes the doctor office has their own sports physical forms, but you can also download and print one from HERE 

• PCHS is offering $25 physicals on May 2 from 2-5pm at PCHS Gym. Bring cash or check and download/print forms HERE to fill out before you go.
